An AI audit trail is the append-only record of how an AI-assisted decision was reached: what the system recommended, which context and evidence it drew on, which human reviewed it, what they changed or overrode, and what the outcome was. It differs from a conventional system log in its subject: a log records transactions, while an AI audit trail records judgment.
Porque importa
Regulated organizations are increasingly required to evidence not just what an AI system produced but that a human meaningfully oversaw it. Most cannot. The recommendation sits in one tool, the approval in a second, the rationale in a chat message and the outcome in a third system, so the chain cannot be reassembled after the fact at any price. An audit trail built as infrastructure records the chain as it happens, which is the only point at which it is cheap to record.
